What if there was no insurance at all? You just pay what teh actual cost of the product or service was? You can shop around to the different "shops" for the best rate and go there for those services. I am sure more people would be getting serviced if it was cheaper because more people are getting serviced. It's like a feedback loop; more people feed into the system, the price goes down for the same service, more people use that service, etc.
The cost of the service is caused by the price the docs command (which is far too high because they have to pay malpractice insurance premiums that are far too high) and/or part prices (which even after the expensive R&D are overinflated with prices).
Maybe healthcare minus insurance should work like 3rd party mechanics and cars.
