When the economy is up you get corporations that want to offer their employees 'better' coverage, or, people that earn more $ want to shop for the best care they feel they can afford. -demand- This is where the 'creep' starts. Slowly and surely the ins co starts managing all your health needs and you let them. From cough syrup to long term care. When the economy turns south lots of folks still feel entitled when the $upply runs out. And here we are.
