On takeoff?

I thought the 16 years ago one was re-entry.

I guess it wasn't as big of a "I still remember where I was" type moment for me because I'm old enough to have that for the first one, which was takeoff. ~1986.
 
Both were ultimately doomed from their take off and bureaucratic asshole fucks. Challenger by the O-ring, Columbia by the foam panel falling off and striking the leading edge of the wing during take off.

The crew of Columbia could have been saved. They had a shuttle nearly ready, and they could have actually examined the wing. Fucking political cowards.
